Release of Liability

In consideration of my child being permitted to participate in the Read It. Make It. Take It. Literacy Workshop, I hereby release, waive, discharge, and hold harmless Right To Read, its officers, directors, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, or damage that may occur to my child or their property during the literacy camp activities, including but not limited to accidents, injuries, illnesses, or damages resulting from negligence or misconduct.

Medical Authorization: I authorize the literacy camp staff to seek and consent to medical treatment for my child in the event of an emergency where I cannot be reached. I understand that every effort will be made to contact me or the emergency contact listed on the registration form prior to seeking medical treatment, but in the event of a life-threatening emergency, immediate medical attention may be necessary.

Photographic Release: I grant permission for photographs or video recordings of my child taken during the literacy camp to be used by Right To Read for promotional purposes, including but not limited to website, social media, and marketing materials. I understand that my child's name will not be used in conjunction with these images without separate written consent.

Compliance with Rules and Policies: I agree to ensure that my child complies with all rules, policies, and instructions provided by the literacy camp staff for the safety and well-being of all participants.

I have read this liability release form in its entirety and fully understand its contents. I acknowledge that I am freely and voluntarily entering into this agreement on behalf of my child.
